Ours hours of operation are:

  • Monday 9:00 a.m. – 5:00 p.m.
  • Tuesday 9:00 a.m. – 5:00 p.m.
  • Wednesday 9:00 a.m. – 1:00 p.m.
  • Thursday 9:00 a.m. – 5:00 p.m.
  • Friday 9:00 a.m. – 5:00 p.m.
  • Saturdays: We are open one Saturday each month. Please call for Saturday hours.

First Visit

Please bring the following items to your initial appointment:

  • Your referral card and x-rays
  • A current medication list (if applicable)
  • Your dental insurance card (if applicable)
  • Photo ID

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs, diabetes, or hypertension).

At your initial visit, we will review an in depth medical and dental health history. Dr. Sogoloff will perform a thorough examination to measure for bone loss, check loose teeth, evaluate occlusion, screen for oral cancer, TMJ, and signs of periodontal disease or gingival recession.

X-Rays

We need current x-rays to diagnose you.  If your dentist has taken x-rays, you may contact them to request that the x-rays be mailed to our office or e-mailed to [email protected]

Cost

The initial examination and consultation start at $125. If x-rays or photographs are necessary, there will be an additional charge.  If dental insurance information is provided, we will submit this visit on your behalf.

An examination and consultation by Dr. Sogoloff are necessary to establish your treatment plan and cost. The fee for periodontal treatment can vary depending on the type of problems and the complexity of treatment. A quote for treatment will be completed at your initial examination and consultation.

Insurance

Dental insurance policies often cover periodontal treatment; however, we cannot guarantee coverage. Patients are responsible for knowing their plan policies and limitations. Please bring all pertinent insurance information to your initial appointment. We will submit dental claims on your behalf.

Upon request, we can submit a pre-treatment estimate for future services after your initial consultation.
